Tags are matched against a stored vocabulary that is only built when you press Learn tags. Adding a tag in the AdminCP afterwards does not update it, so the new tag can never be chosen — and because the app still picks the closest match from the tags it does know, the result is topics tagged with something unrelated rather than an obvious failure.

Nothing warns that the vocabulary is out of date.
